The Elder Guardian 360™ Family Portal provides approved users with access to selected updates, appointment information, non-clinical visit notes, documents, and care-coordination information related to an active client’s services.

Access Family Portal

Portal access is limited to active clients and individuals authorized by the client or authorized representative.

Important: Portal content and available features depend on the client’s selected service plan, signed authorizations, portal configuration, and information available for sharing. The portal is not continuously monitored and must not be used for medical emergencies, urgent safety concerns, or time-sensitive requests. For an emergency, call 911.

What May Be Available in the Family Portal

Portal content is personalized and may vary according to the client’s service plan, permissions, and the type of support being provided.

Visit Updates

Review approved summaries and non-clinical observations from scheduled Elder Guardian 360™ visits when included in the client’s service plan.

Appointment Information

Access selected appointment dates, preparation notes, summaries, provider instructions supplied during visits, and organized follow-up information.

Care-Coordination Updates

View authorized information about scheduling, services, transitions, pending questions, family tasks, and agreed next steps.

Non-Clinical Wellness Observations

Review general, non-clinical observations that have been approved for family sharing. These are not medical assessments or nursing evaluations.

Approved Documents

Access selected forms, summaries, resource information, service documents, or other files made available to authorized portal users.

Organized Family Information

Keep authorized family members connected through one organized location for approved updates and documents related to the client’s support.

Families often live in different cities, have demanding schedules, or are unable to attend every appointment and visit.

The Family Portal can provide one organized location for selected updates, documents, appointment information, and care-coordination details that the client has authorized for sharing.

The portal supports communication and organization, but it does not replace direct conversations, medical records, emergency services, or the instructions of licensed professionals.

Privacy, Access & Confidentiality

Portal access is limited to the active client and individuals authorized by the client or authorized representative. Users should not share login details, portal links, downloaded documents, or client information with unauthorized individuals.

Information made available through the portal is handled according to the Elder Guardian 360™ Privacy Policy, applicable authorizations, service agreements, and operational requirements.

Elder Guardian 360™ may retain non-clinical service notes, appointment information, wellness observations, family communications, service documents, and client-provided information. We do not create or maintain official clinical medical records.

No website, email service, cloud platform, or electronic system can be guaranteed to be completely secure. Authorized users are responsible for protecting their devices, passwords, downloads, and access credentials.

The portal is not an emergency communication, medical-monitoring, or 24-hour response service.
