Initial Care Assessment
Required Before Monthly Plan Enrollment
Every new client who wishes to enroll in an ongoing monthly advocacy plan must first complete a personalized Initial Care Assessment. During the assessment, we review the current situation, identify immediate concerns, discuss communication and coordination needs, answer questions, and determine which level of monthly support may be the most appropriate fit.
The Initial Care Assessment is required for monthly advocacy plans. Stand-alone coaching and Transition & Urgent Support have separate enrollment processes.
Pay & Schedule Your AssessmentMonthly Advocacy Plans
Ongoing support options for older adults and families who need different levels of communication, organization, advocacy, and care coordination.
Complete the Assessment Before Enrolling
Step 1: Pay, schedule, and complete the $199 Initial Care Assessment.
Step 2: After the assessment, Elder Guardian 360™ will review your needs, confirm the most appropriate monthly plan, and provide enrollment instructions.
Step 3: Use the private enrollment link provided by Elder Guardian 360™. When enrollment is completed within seven calendar days after the assessment, the full $199 assessment fee will be applied as a one-time credit toward the first plan payment.
Monthly plan payment links are not available publicly. Each plan button below begins with the required assessment. After the assessment is completed and the appropriate plan is confirmed, Elder Guardian 360™ will send a private plan-enrollment link.
