📄 Power of Attorney (POA) Support & Coordination
Power of Attorney (POA) Support & Coordination is designed for clients and families who have already established a valid POA and need assistance acting within that authority.
What This Service Includes:
Secure intake and verification of existing POA documentation
Review of authority scope for service alignment
Coordination and advocacy as permitted by the POA
Communication with family members and approved third parties
Documentation, reporting, and risk monitoring
Acting strictly within the authority granted

All Power of Attorney documents must be prepared and executed through a licensed attorney or appropriate legal professional.
Services requiring POA authority may only begin after valid documentation is received and verified.
🔐 Authority & Documentation Requirement
Services requiring legal or medical authority cannot be provided until valid documentation is submitted and verified.
Elder Guardian 360™ reserves the right to limit, suspend, or discontinue services if required authority documents are unavailable, incomplete, revoked, or disputed

Setup fees are non-refundable and required before authority-based services begin.

Add-On Visit Time Policy (Only If a Visit Runs Over 2 Hours)
Each add-on service includes up to two (2) hours of visit time unless otherwise stated.
Additional visit time is charged only if the visit exceeds two (2) hours, billed in 30-minute increments:
30 minutes: $50
60 minutes: $85
Additional time must be approved by the client or authorized family member/representative on file.
Important Notes
We are a non-medical support service (no skilled nursing or medical care).
Packages are billed monthly. Add-ons are billed per service.

Authority-Based Support Add-Ons
Power of Attorney & Health Care Surrogate Support
Elder Guardian 360™ offers authority-based support add-ons for clients who have legally executed documents in place and require professional coordination, advocacy, and oversight.
These services are available only as add-ons to an active Elder Guardian 360™ care plan
🏥 Health Care Surrogate Advocacy & Coordination
Health Care Surrogate Advocacy & Coordination provides non-medical support, communication, and presence as authorized by an existing Health Care Surrogate designation.
What This Service Includes:
Verification of Health Care Surrogate documentation
Communication and coordination with medical providers as permitted
Appointment or hospital presence (as authorized)
Family updates and documentation
Advocacy and coordination support
Recordkeeping and reporting

Elder Guardian 360™ does not provide medical care, treatment, or medical decision-making.
All services are limited to communication, coordination, and advocacy as permitted by law and the Health Care Surrogate designation on file.

Authority-based services carry increased responsibility and risk and are priced separately from care plans.
