Initial Care Assessment

$199

Required Before Monthly Plan Enrollment

Every new client who wishes to enroll in an ongoing monthly advocacy plan must first complete a personalized Initial Care Assessment. During the assessment, we review the current situation, identify immediate concerns, discuss communication and coordination needs, answer questions, and determine which level of monthly support may be the most appropriate fit.

How the $199 Credit Works: Book, pay for, and complete the assessment first. If you enroll in an approved monthly Elder Guardian 360™ advocacy plan within seven calendar days after the assessment, the full $199 assessment fee will be applied as a one-time credit toward your first plan payment. No public coupon code is required. You will receive a private enrollment link and credit instructions after the assessment.

The Initial Care Assessment is required for monthly advocacy plans. Stand-alone coaching and Transition & Urgent Support have separate enrollment processes.

Step 1: Pay, schedule, and complete the $199 Initial Care Assessment.

Step 2: After the assessment, Elder Guardian 360™ will review your needs, confirm the most appropriate monthly plan, and provide enrollment instructions.

Step 3: Use the private enrollment link provided by Elder Guardian 360™. When enrollment is completed within seven calendar days after the assessment, the full $199 assessment fee will be applied as a one-time credit toward the first plan payment.

Monthly plan payment links are not available publicly. Each plan button below begins with the required assessment. After the assessment is completed and the appropriate plan is confirmed, Elder Guardian 360™ will send a private plan-enrollment link. Monthly advocacy services begin only after enrollment in the confirmed plan has been finalized.

Purchase Four-Week Coaching
Coaching scope: Coaching provides education, planning, decision support, practical guidance, and accountability directly to the client or family. It does not include provider or facility communication, appointment accompaniment, active care coordination, emergency assistance, or unlimited access. Between-session email support is limited to the brief check-ins stated in the selected package and is answered during regular business hours, generally within one business day.

Advocacy-plan coaching add-ons: Add-on prices are discounted exclusively for clients enrolled in an active Elder Guardian 360™ monthly advocacy plan. Stand-alone coaching packages are priced separately and do not require monthly advocacy enrollment.

Transition & Urgent Support

Focused, scheduled, non-medical advocacy and care coordination during a hospital stay, rehabilitation stay, discharge, relocation, caregiver absence, sudden change in care needs, or another time-sensitive transition. These services do not include emergency response or 24-hour availability.

Urgent Support Engagement

$250

Minimum Engagement • Includes Up to 2 Hours

Focused advocacy, organization, communication, and coordination for families who need help with a hospitalization, rehabilitation stay, discharge concern, sudden care change, urgent meeting, or another limited time-sensitive need.

The initial review and immediate needs assessment are included in the first two hours. Additional service time is billed at $125 per hour with prior client approval. Calls, provider or facility communication, care coordination, documentation, research, appointment participation, family updates, and applicable travel time count toward service hours. Service is subject to location, authorization, scheduling, current capacity, and the agreed scope. Same-day assistance is not guaranteed.

Important: Elder Guardian 360™ provides non-medical senior advocacy, care coordination, coaching, appointment assistance, organizational support, non-clinical wellness observations, and authorized family communication. We do not provide medical treatment, nursing care, medication administration, hands-on personal care, guardianship, legal advice, financial management, psychotherapy, emergency response, continuous supervision, or 24-hour monitoring.
