Coaching scope: Coaching provides education, planning, decision support, practical guidance, and accountability directly to the client or family. It does not include appointment accompaniment, provider or facility communication, active advocacy, emergency response, or 24-hour availability.

Families enrolled in a monthly advocacy plan may add coaching for additional education, planning, guidance, and accountability.

What if I do not need monthly advocacy?
Coaching may be a good fit when you primarily need education, planning, problem-solving, and help deciding what to do next without ongoing advocacy or care coordination.
Is coaching the same as therapy or counseling?
No. Coaching is not psychotherapy, mental-health counseling, medical treatment, or crisis counseling. It focuses on practical education, planning, decision support, and action steps.
Can family members participate?
Yes. Adult children, spouses, caregivers, or other involved family members may participate when appropriate and with the permission of the person receiving coaching.
Can coaching be completed virtually?
Yes. Sessions may be completed by telephone or video. In-person sessions may also be available depending on location, scheduling, and the selected service.
Does coaching include contacting providers or facilities?
No. Coaching primarily provides education and guidance directly to the client or family. Communication with providers, facilities, or outside services is generally part of advocacy or care coordination rather than coaching.
What does between-session email support include?
Between-session email support includes up to two brief coaching-related check-ins between scheduled sessions when that support is included in the selected package. Messages are answered during regular business hours, generally within one business day. It does not include emergency assistance, extensive research, provider communication, care coordination, or unlimited messaging.
Does the Four-Week Transition Coaching Intensive renew monthly?
No. It is a one-time four-week coaching program that includes four scheduled 60-minute sessions, a written transition and action plan, limited between-session email check-ins, and a written closing summary. It does not renew automatically.
What happens after coaching?
You will leave with clearer direction, practical recommendations, and next steps. When more active advocacy or care coordination is needed, we can discuss available support options.

Both services provide guidance and support, but they are designed for different levels of involvement.
Senior Life & Family Coaching
Coaching is appropriate when you need education, encouragement, planning assistance, and help working through decisions.

Short-Term Advocacy & Coordination
Transition & Urgent Support provides scheduled, short-term, non-medical advocacy and care coordination during time-sensitive, unexpected, or significant care transitions.
- Hospital and rehabilitation transitions
- Unexpected changes in health or circumstances
- Time-sensitive family care situations
- Discharge and transition-planning support
- Authorized communication with providers and facilities
- Hourly support with a two-hour minimum
- A 30-day option that includes up to 10 hours of scheduled support during a service period of up to 30 consecutive calendar days
- Additional service time available at $125 per hour with prior approval
- Organization of information, resources, and next steps
Best for:
Families navigating a hospitalization, rehabilitation stay,
discharge, sudden care change, or significant life transition.
Transition & Urgent Support does not include unlimited access, daily visits, on-call availability, emergency response, or 24-hour support. Unused package hours expire at the end of the applicable service period and do not roll over.
